Report Card:
Wood Lane Residential Services, Inc prides itself on providing quality services. We have identified key areas to analyze to help maintain the Mission, Vision and Values of our organization. We feel that this is accomplished by looking at all aspects and all departments. The Report Card will reflect the data collected then identify related goals.

2008 will serve as a baseline for future report cards. This year the information reported will just report on where we are today from which we will identify the measurable goals for 2009.

Data will be entered in the report card 2 x year (approximately January and July). New goals will be rendered from the result of previous years performance and from new “hot spots” identified for each department.


Quality Assurance .

WLRS,Inc has always taken great pride in the quality of our services. We have very high standards, which is why many families have entrusted the care of their loved one to us throughout the years. We maintain a positive reputation among families of consumers, the community, the Wood County Board of DD and the Ohio Department of DD for being a residential provider that follows our mission statement and provides excellent quality care to all who receive our residential services.

How is WLRS, Inc. different from any other residential provider in Wood County and the state of Ohio? WLRS has a dedicated and specific quality assurance department that continuously oversees compliance of federal and state regulations, as well as policies and procedures for the agency. If there are identified deficit areas that are discovered by this department, WLRS will ensure that these issues are resolved immediately, specifically to ensure health and safety of all. Furthermore, the quality assurance department may develop a plan of good practice, if necessary, that is communicated throughout the program for all to follow.

Quality Assurance is maintained through the following practices:
  • Facility walk thru checklists
  • Maintenance checklists
  • Facility quality assurance checklists (review of finances, emergency drills, staff certification and training, unusual incident reports)
  • Medical record checklists
  • Skill development plan reviews
  • Individual plan checklists
  • Home observations
  • Unusual incident report follow up (for each unusual incident reported)
