ICF/MR
- As openings occur, we receive referrals from the Wood County Board of DD.
- An individual eligible for placement in
an ICF must have the following:
- The need for continuous active treatment
- Are eligible for Medicaid
- Have a mild, moderate, severe or profound mental retardation diagnosis
- Individuals/ families interested in a possible ICF/MR placement will tour the home
- If the family/ individual is interested after the tour, they will visit the home. Prior to the visit the Service Coordinator will get a Behavior Assessment, Current IP or Service Needs Assessment, Consent for Mutual Disclosure and Emergency Information to the home.
- The individual will then complete 2-3 successful visits in the home setting, ideally with one being an overnight visit.
- If the individual/
family is still interested, the following collateral will be
collected
- Psychological Evaluation or Multifactor Evaluation current within 3 months (evaluator must hold state license)
- Physical
current within 1 year including
- Written results of testing for Hepatitis B Antigen and Antibody
- Written result of TB testing (Mantoux PPD or chest x-ray)
- Cervical spine x-ray for individuals with Down’s Syndrome
- IEP/IP current within 1 year (if applicable) and Service Needs Assessment
- Medical/ Social History Questionnaire current within 1 year
- Behavior Assessment
- Miscellaneous information
- A preadmission meeting will be scheduled to review the visits, collateral and collect additional information as needed.
- After the meeting, the summary of the needs will be forwarded to the CEO.
- The CEO will forward this summary to the Superintendent of the Wood County Board of DD.
- The CEO will notify the preadmission team of the placement decision.
- A date will be determined for the individual to move into the home.
