Less than 24-hour services (Drop in)
- The Wood County Board of DD will contact Wood Lane Residential Services, Inc if there is an individual/ family interested in receiving drop in services.
- And individual/ family will bee with the Associate Director or designee to identify services the family is interested in receiving.
- The Associate Director or designee will tour the home/ facility in which services will be delivered.
- If the family/ individual would like
WLRS, inc to provide the service, WLRS, inc will need the following:
- Behavior Assessment (current within 1 year)
- Consent for Mutual Disclosure with WCBDD
- Psychological Assessment
- Medical/ Social History current within 1 year
- IP/ Service Needs Assessment
- The collateral and the assessment of the tour will be reviewed and a summary will be submitted to the CEO.
- The CEO will make a determination for or against service provision by WLRS, inc.
- An IP
or Amendment will identify the services WLRS, inc is to
provide prior to commencement of services.
